What are Care Management Services?
Care management services are delivered by licensed professionals and are sometimes also referred to as Targeted Case Management (TCM). A care manager generally first assists with identifying strengths, needs, goals, and desires through a person-centered treatment planning process. They may help with referrals and provide support in coordinating with community agencies.
Throughout treatment, the care manager monitors progress toward treatment goals and engages in supportive problem solving with challenges that may arise. Additionally, there is a continual focus on supporting the whole person in overall physical health and wellness, family involvement, community integration, and achievement of personal goals.
